The Grady Heart Failure Program: A Model to Address Health Equity Barriers

This new tool, developed by CDC in partnership with the Grady Health System, can support health care organizations address barriers to improve the health outcomes of heart failure patients. The Grady Heart Failure Program is located at Grady Memorial Hospital in Atlanta, Georgia, and was designed to address socioeconomic barriers to care at the individual level.

This guide provides a detailed description of an intervention intended to address health disparities among heart failure patients.
